Presenter Bio - Exploring the Role of Families, Communities, and Public Health in Implementing the Medical Home System

Eileen Forlenza, BS

Eileen Forlenza serves as the Director of the Colorado Medical Home Initiative for the Department of Public Health and Environment . Ms. Forlenza serves on state and national advisory councils including National Summit on Patient and Family-Centered Care, National Center on Cultural Competence, Multi-Stakeholder Patient Centered Medical Home Pilot Project, National Initiative for Child Health Quality, Association of Maternal and Child Health and Family Voices National. Eileen has held Governor appointed positions on the Colorado Interagency Coordinating Council and the Colorado Developmental Disabilities Council and served as a Colorado delegate to attend the national Family Policy Academy. Eileen holds credentials in family leadership development, including curriculum design and implementation.

Her extensive experience also includes social entrepreneurship developing and implementing initiatives that serve at-risk populations such as children with disabilities, siblings in foster-care, domestic abuse victims, incarcerated women, as well as training a national school-based network of youth leaders. Eileen's social marketing efforts have generated awareness in national forums such as People magazine, Family Support magazine and "Oprah!"
